Mifflin County Soccer Club Fall 2008 Registration

Division coordinators will place players on teams, an attempt will be made to accommodate special requests and/or needs, however the coordinators’ decisions are final.
Cost of registration is $30 per player up to a maximum of $70 per household. Make checks payable to: Mifflin County Soccer Club (There will be NO REFUNDS)
Age brackets begin at 5 years of age and extend through the 6th grade (maximum age is 13, We encourage participation in the Jr High program for 7th & 8th graders)
4 year olds may play only if a parent/guardian is registered as Head Coach or Assistant Coach

Registrations may be returned by mail to MCSC, P.O. Box 663, Lewistown, PA 17044 or turned in at Total Life Care Pharmacy at the Lewistown Hospital from April 14th, 2008 to June 14th, 2008.

Realizing that no athletic activity, including soccer, is without risk of injury to the participants, I the Parent/Legal Guardian give permission for him/her to participate in the MCSC Recreational League and release Mifflin County Soccer Club and all sponsors, officers, coaches and officials from any and all liability.
